Report Security Incident

What is a Security Incident?

A security incident includes any event that may compromise the confidentiality, integrity, or availability of university data or systems, such as:

  • Phishing emails or suspicious messages asking for credentials
  • Compromised accounts (unauthorized access to your email, systems, or data)
  • Lost or stolen devices containing university data
  • Malware or ransomware infections
  • Unauthorized access to sensitive information
  • Data breaches or accidental disclosure of protected information
  • Suspicious system activity or unusual network behavior

Reporting a Security Incident

To report a security incident, click on the button to the right. Be prepared to provide the following information:

  1. Name & Contact information (email, phone number, office #) - This should be entered automatically
  2. Date and time the incident took place
  3. Description of what took place
  4. What system(s) were affected
  5. Any actions that have already been taken

This will submit a Service Request within I.T.'s ticketing system and someone will follow up with you as soon as possible.
